Verify the exact plan
An insurer's name alone does not confirm network participation or coverage. Plans from the same company can use different networks and rules. Have your insurance card ready when you contact the office and ask how to confirm whether the clinician and service are covered under your specific plan.
Then check with the member-services number on your card. Ask about specialist benefits, referral requirements, and any restrictions that apply to the visit. Save the date and reference number of the conversation if your insurer provides one.
Separate the consultation from a treatment
Benefits for an office visit can differ from benefits for a procedure, medication, laboratory test, or imaging study. If a treatment is recommended, ask for the exact service information needed to check coverage. Some services may require prior authorization or may be excluded by a plan.
Authorization, a benefit estimate, and payment of a final claim are different steps. Ask what your deductible, copayment, or coinsurance may mean for the specific service. Avoid assuming that a general statement about coverage applies to every treatment.
Ask for a clear cost conversation
If you expect to pay directly or a service is not covered, ask about the current price and what it includes before agreeing to care. Clarify whether follow-up, medication, supplies, outside testing, or another clinician's fee is separate. This website does not publish a universal consultation price or guarantee a payment arrangement.
If a recommended plan is financially difficult, say so early. The clinical conversation can include appropriate alternatives, and the office can explain the current administrative options. Do not assume that a medicine advertised elsewhere will have the same price through your plan or pharmacy.
Bring the information that prevents delays
Bring a current insurance card, identification, medication list, and any referral or authorization information your plan requires. Ask the office how records should be sent before the visit. If your coverage changes, notify the office rather than relying on a card already on file.

Your questions, answered
A little more clarity.
Do you accept my insurance?
Contact the office with your exact plan information, then confirm participation and benefits with your insurer. We do not list unverified plan acceptance here.
Will insurance cover PRP, hormones, or weight-management medication?
Coverage depends on the exact service, product, indication, and plan rules. Check each proposed item before treatment.
Does an approved referral guarantee payment?
No. Referrals, authorization requirements, benefits, and final claims decisions are separate. Ask your insurer about your specific responsibilities.
