General contact requests

When you send our contact form, the name, email address, optional phone number, and message you enter are sent to the practice’s contact-request service so the team can respond. The form also includes technical information used to help limit unwanted submissions.

Use this form for general questions about appointments or services. Please do not include medical records, detailed health information, insurance identifiers, payment-card details, or completed intake forms. It is not an emergency or clinical messaging service.

Patient forms

The PDFs on our patient resources page can be completed in your browser. Entries stay in the viewer until you choose to submit them. Submitting sends your completed PDF, name, and email address to the practice’s shared portal, where authorized staff can review the submission and access the privately stored attachment. Downloading a copy alone does not send it to our team. Closing the viewer clears unsent entries; you can download a copy before leaving.

Scheduling and payments

Online scheduling is provided through Tebra. Payment links open Stripe. Information you enter into those services is handled within their systems and under the notices they provide. Opening maps, email, or another external link also takes you to a separate service.

Embedded services and external sites may use cookies or similar browser storage. Your browser’s settings can limit this storage, although some features may work differently.

Technical information

Hosting and request services may process technical information, such as connection and request details, to deliver pages, operate forms, and manage unwanted traffic. Contact the practice if you need more detail about a particular service or a request you submitted.

Cloudflare Web Analytics

We use Cloudflare Web Analytics to understand page visits and website performance. Cloudflare receives page and technical performance information through a script on our public website. This service does not use analytics cookies or browser fingerprinting. Our integration does not send contact-form entries, completed patient forms, or booking details to Cloudflare Web Analytics.
