Downloading a form does not send it to the practice. Ask our team how to return completed documents. The original new-patient packet includes a previous address; use the current contact details on this website and confirm your visit location with the practice.

The payment link opens Stripe. Check the details in that service before paying, and contact the practice with questions about your balance. A payment does not confirm an appointment or a particular treatment.
